Application of Result Slip

Please read all of the below information to ensure that your result slip request is processed correctly.


This Result Slip Requisition Form ("Form") is only designated for current session candidates, who are eligible to apply for ONE official result slip for free within two months from the result release date (i.e. on or before 19 October 2021 for Final Examination).


This Form is not suitable for requesting:

(i) additional copies of result slip;
(ii) result slip after the two-month period; or
(iii) result slip of previous sessions.

An administration fee of HK$100 is required for the above requests. Please call 2287 7228 for details. 


Result slip will normally be ready in 10 working days after this Form is received. Candidate can choose to pick up in person or receive it via ordinary mail to the provided address. No courier service or registered mail will be provided. 


Candidate is responsible for ensuring that the correct and clear information is submitted. Please review the information provided in this Form prior to submission to avoid delay or additional charge. 


The Institute is not responsible for delay in receipt or loss of result slips in mail. Make sure the mailing address is correct to avoid re-ordering of result slip and incurring additional administration fee. 


Candidate can authorise another person to pick up the result slip on his/her behalf. An authorisation letter, stating the name and HKID Card or Passport number of the authorised person is required upon collection. 


Result slip will not be issued to candidate who has an outstanding fee payable to the Institute. 


The information provided in this Form will only be used for the processing of this requisition of result slip. 


Any result slip issued under this Form but not collected will be destroyed after three months from the date of request.  Candidate not collecting his/her result slip have to apply for a re-issued copy by calling 2287 7228 and pay an administration fee of HK$100.
